Your browser and network decide which protocol to use for each connection. This page shows whichever one your request actually arrived on. Seeing both at once would require two separate endpoints - one reachable only over IPv4 and one only over IPv6 - which most single-domain sites, including this one, don't run.
IP-based location is approximate - it's usually accurate to the city or region your internet provider routes through, not your exact address.
Private, loopback, and reserved IP addresses (e.g. on a local network or behind certain VPNs/proxies) have no public location or reverse DNS record to look up.
